Abstract

Abstract. Wound healing is a complex biological process encompassing inflammatory, proliferative, and remodeling phases. Red betel (Piper crocatum) is rich in bioactive compounds for tissue repair. Objective: To analyze the in vivo efficacy of its topical formulations (ointment, cream, patch) on wound healing. Methods: Literature synthesis of five experimental rodent studies evaluating macroscopic and histopathological parameters. Results: Secondary metabolites (flavonoids, tannins, saponins, alkaloids) act synergistically as anti-inflammatory, antioxidant, and antibacterial agents (S. aureus, E. coli). Ointments (15%–45%) and cream (15%) suppressed acute inflammation, prevented pus, and accelerated wound closure by day 14. Combination transdermal patches (30%) enabled sustained release, significantly enhancing fibroblast proliferation (>50 cells/field) and dense collagen deposition. Conclusion: Red betel holds strong potential as a natural topical agent, where vehicle optimization is key to wound repair.

Abstrak. Penyembuhan luka merupakan proses biologis kompleks yang mencakup fase inflamasi, proliferasi, dan remodeling. Daun sirih merah (Piper crocatum) kaya senyawa bioaktif berpotensi memacu pemulihan jaringan. Tujuan: Menganalisis efektivitas sediaan topikal ekstrak sirih merah (salep, krim, patch) terhadap penyembuhan luka secara in vivo. Metode: Sintesis data dari lima studi literatur eksperimental pada tikus Wistar dan mencit dengan pengamatan makroskopis dan histopatologi. Hasil: Metabolit sekunder (flavonoid, tanin, saponin, alkaloid) bekerja sinergis sebagai antiinflamasi, antioksidan, dan antibakteri (S. aureus, E. coli). Salep (15%–45%) dan krim (15%) efektif menekan inflamasi akut, mencegah pus, serta mempercepat penutupan luka pada hari ke-14. Patch transdermal (30%) memberikan pelepasan terukur (sustained release) yang memicu proliferasi fibroblas (>50 sel/lapang pandang) dan pembentukan kolagen padat. Kesimpulan: Sirih merah berprospek tinggi sebagai obat topikal alami. Optimasi bentuk sediaan menjadi kunci percepatan pemulihan luka.
